New Jersey VFC provides public purchase vaccines for eligible children at no cost (including shipping) to public and private providers. Included are vaccines recommended by the Advisory Committee of Immunization Practices and approved by the Centers for Disease Control and Prevention to protect against the following diseases:
- Diptheria
- Tetanus
- Pertussis (Whooping Cough)
- Polio
- Measles
- Mumps
- Rubella (German Measles)
- Haemophilis influenzae type B
- Pnuemococcal Disease
- Hepatitis B
- Varicella (Chickenpox)
- Hepatitis A
New Jersey VFC saves parents and enrolled providers out-of-pocket expenses for the vaccine. (Note: Providers may still charge for office visits and administration.)
Provider enrollment is easy! Simply contact the New Jersey Department of Health and Senior Services at 609-588-7512 and you will be sent an enrollment packet. Upon enrollment, you will need to analyze the number of VFC-eligible children in your practice so VFC can determine the funding needed to purchase vaccines.
Once enrolled, providers are notified of their PIN (provider identification number) and other information by mail. Then vaccines can be ordered from Automated Health Systems by phone, fax, mail or via Internet e-mail.
Materials, including Vaccine Information Statements, the Vaccine Eligibility/Encounter Record, an order form and similar program forms are mailed automatically when a vaccine order is placed by an enrolled provider.
