Most families with children who receive their health care through the Medicaid and BadgerCare program are enrolled in an HMO. Mandatory or voluntary HMO enrollment is based on HMO participation by zip codes, also known as "service areas".
- Mandatory Enrollment: recipients reside in a zip code area where two or more HMOs provide Medicaid and BadgerCare coverage.
- Voluntary Enrollment: recipients may voluntarily enroll into an HMO if only one HMO is available in a zip code area.

An initial HMO enrollment packet is sent to Medicaid and BadgerCare recipients within 10 days of eligibility determination. The packet of materials includes a welcome letter; the Enrollment Choice Form (which lists the HMOs available for consumers to select); the Choosing Your HMO booklet (which describes the HMOs); A Guide to the Wisconsin Medicaid and BadgerCare HMO Program and an envelope to mail the Enrollment Choice Form.

Enrollment into an HMO is simple and quick and may be done through the mail, by telephone or in person.
To Enroll by Mail:
- Complete the Enrollment Choice Form provided in the enrollment packet.
- Mail in the Enrollment Choice Form using the envelope included in the Enrollment Packet.
To Enroll by Phone:
- Call the Enrollment Specialist at 1-800-291-2002 and enroll right over the phone.
- The Enrollment Specialist is available Monday through Friday, 7:00am until 6:00pm.
- Bilingual Hmong and Spanish Enrollment Specialists are available.
- Hearing impaired services and other translation services are available.
In Person:
- Meet face-to-face with an Enrollment Specialist at various locations throughout the State.
